Appointment Cancellation/ Reschedule/ No-Show/ Late Policy
We understand that sometimes it is necessary to reschedule or cancel an appointment; however, we require clients to provide us with at least 24 hours notice (within business hours) if you are unable to keep the time we have reserved for your appointment.
Out of respect for your time and ours, no-shows, cancellations, or reschedules less than 24 hours before any appointment will be charged if this continuously reoccurs.
If a client has canceled, rescheduled and/or not shown three times, the client will then be required to prepay in full for future services at time of booking appointment. The amount pre-paid for is nonrefundable if the policy is violated. If the appointment is rescheduled prior to 24 hours of the appointment time, the amount paid will be credited for future services at Imagine Institute.
Arriving late will deprive you of valuable treatment time. If you are more than 15 minutes late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply if reoccurring.
We regret any inconvenience this may cause. We appreciate your understanding of our cancellation/ reschedule/ no-show/ late policy. This will allow the Imagine Institute team to continue to provide the highest quality service for you and future patients, as well as valuing our team members’ time.
By scheduling an appointment, you are agreeing to our cancellation/reschedule/ no-show/ late policy.
Returns Refunds
ALL SERVICES SALES ARE FINAL. We do not offer refunds on any services rendered. We do not offer refunds on any used or unused procedures, service packages or series. We practice a strict no refund policy on in-store services, packages and membership dues.
Imagine Institute accepts returns of unused products in their original packaging purchased in-store or from our online store (https://www.imagineinstituteboca.com/shop) for a full refund within 14 days with an original receipt.
We do not offer refunds on retail products purchased. Defective products may be exchanged within 14 days for the same product only.
Gift Cards
Please note that Imagine Institute gift cards may not be returned and should be treated like cash. They cannot be replaced if lost or stolen, but they never expire. They cannot be redeemed for cash unless required by law.
Financial Responsibility Agreement/Payment Policy
As a client of Imagine Institute, you understand that you are responsible to pay in full at the time any service is rendered. We accept cash, most major credit or debit cards (VISA, MasterCard, Discover, American Express), Zelle, Venmo and/or PayPal. We also offer financing through Care Credit or Cherry. We do not accept checks or insurance. We require you to maintain an active credit/debit card on file.
The prices on your customized treatment program will be valid for 1 month (30 days). Prices are subject to change after 30 days. Time-sensitive promotions may end prior to the 30-day window. To guarantee the price, you may pay 50% in advance for any procedures. All sales are final and cannot be refunded. All pre-purchased services must be used within 1 year from purchase date.
If you have received treatments from your package and choose to break up the package, the treatments received will revert to full price and the difference will be credited to your account. The remaining funds will be available to use toward other services and products at Imagine Institute. We require that all services sold in packages be used within 1 year of purchase date.
I clearly understand and agree that all services rendered to me may be charged directly to me, and that I am personally responsible for full payment. I understand that even if I suspend or terminate treatment, any fees for professional services rendered to me or to my dependent up to the point of termination will be immediately due and payable.
I acknowledge that I am responsible for any outstanding fees for services provided to me by Imagine Institute.
Any other arrangements that may involve payment plan or payment deferral must be made in writing with Imagine Institute. Verbal agreements are not acceptable.
I acknowledge that the Imagine Institute reserves the right to charge a fee if I do not attend or cancel the scheduled appointment without providing 24-hour notice. I further acknowledge that the Practice reserves the right to reschedule my appointment if I am more than 15 minutes late to the scheduled appointment.
